In 2022, nearly 110,000 people died of drug overdoses, according to the most recent CDC report. The preliminary count of 109,680 overdose deaths was only slightly higher than the figure for 2021 when 109,179 people were estimated to have died. The years 2020 and 2021 saw overdose-related deaths spike by 30% and 17% respectively.
Despite the grimmest era of drug addiction and overdose death in our history, healthcare providers are struggling to implement innovative programs to address the crisis as they do not have the right blend of good treatment supported by innovative technology and data analytics.
The Hotspotting the Overdose Epidemic Program (The Hotspotting Program) is an example of an innovative initiative utilizing technology and data to prevent overdose. The program aims to leverage predictive analytics to identify individuals at the highest risk of overdose and engage them before an adverse event, using a person-centered, peer-led care management model. Initial program evaluation results showed an 81% reduction in non-fatal overdoses in the pre-post period analysis for those clients engaged in the program. More importantly, there were two overdose-related deaths compared to eleven in the non-engaged comparison group.
This editorial discusses the critical technological innovations leveraged in this program to prevent overdose cost-effectively.
1. Predictive Analytics to Move from Reactive to Proactive Outreach:
The use of predictive analytics in the Hotspotting Program marks a critical paradigm shift from reactive to proactive outreach. Participating treatment providers in the Hotspotting Program use an AI/machine learning model and data-sharing arrangements to identify those clients most at risk of a future overdose and engage them before a crisis occurs. Given the increased lethality of the nation's illicit drug supply, a proactive approach is crucial.
The SI PPS developed the AI/machine learning model in partnership with the Massachusetts Institute of Technology's Sloan School of Management to predict overdose incidents. The model incorporates 107 predictor variables and one outcome variable. The predictor variables are motivated by known risk factors for opioid-related harm, including diagnostic codes, demographic information, provider encounters, and prescriptions filled. The outcome variable considers any events associated with opioid misuse, overdose, and fatal overdose. The model was trained using the SI PPS's comprehensive database of medical claims data, electronic health records (EHR) data, and social data collected through data partnerships with emergency health, criminal justice, and social service agencies.

2. Common Digital Platform to Support Comprehensive Care Management and Referrals
A common digital platform plays a pivotal role in the success of the Hotspotting Program by facilitating peer-led care management and referrals across behavioral, medical, and social services.
This shared platform ensures that relevant information is easily accessible to peer-advocate care managers, healthcare professionals, and community-based organizations. It enables them to collaborate efficiently and to provide the necessary comprehensive care focused on reducing opioid harm, promoting a continuum of recovery, and addressing critical social determinants of health factors. The streamlined communication and coordination enhance the overall effectiveness of the program. Importantly, the sightline into the overall program performance by the program director allows for early intervention in underperforming domains. By incorporating a social determinant of health screener and referral aspect, the platform allows for a 360-degree view of a recovery trajectory, a substantial improvement over traditional record management.
3. Real-Time Data for Program Performance Monitoring:
Real-time data is a vital component of the Hotspotting Program, enabling continuous program performance monitoring and optimization. This data-driven approach allows program managers and treatment providers to adapt and optimize their strategies based on real-world activities and outcomes. Monitoring the program in real-time ensures that gaps are identified and resolved early and interventions remain effective and responsive to the evolving landscape of the overdose crisis, ultimately contributing to a more agile and impactful initiative. As information becomes available via multiple inputs, such as EMS reports of overdose, hospital ER, and inpatient visits, the calculus of engaging with an individual can be changed on the fly without waiting for long periods of noncommunication of critical events.
4. Contingency Management Delivered Through a Client-Facing Mobile Application:
The integration of a contingency management protocol for substance use disorder through a client-facing mobile application represents a user-centric approach in the Hotspotting Program. Contingency Management is an evidence-based behavioral therapy that provides tangible reinforcers for evidence of behavior change related to recovery. This proven intervention is effective in increasing treatment retention rates and reducing substance use in a cost-effective way. In harnessing the power of Contingency Management through a mobile application, the Hotspotting Program not only embraces innovative technology but also empowers individuals to engage actively in their own care. The mobile application serves as a multifaceted tool, allowing clients to access educational resources, track their progress in meeting treatment goals, and receive timely feedback. This comprehensive and personalized approach fosters a sense of agency and accountability in preventing overdose, showcasing the potential for technology-driven interventions to make a meaningful impact on the trajectory of substance use disorder treatment.
